Many patients use medical cannabis as an alternatives, or in addition to, pharmaceutical medications to assist with symptoms relief caused by their qualifying health condition. Cannabis is typically well-tolerated and many patients are pleased to discover a natural and effective alternative to relying on pharmaceuticals alone. We are happy to assist patients by providing cannabis education and medical evaluations to determine if medical cannabis is appropriate for you and your condition.
We have helped hundreds of patients navigate the process of obtaining their 329/medical cannabis cards in Hawaii. Our staff can assist you with creating your application with the Hawaii Department of Health and will evaluate your condition using your existing medical record to determine whether you qualify for medical cannabis. If you have not had your condition diagnosed by a doctor previously, we can assist with this as well (please note this will be a longer appointment and may require obtaining your medical records/labs/imaging. )
We are a mobile clinic and offer house calls throughout the island of Maui.
Initial appointments can be conducted virtually if you have medical records verifying your condition. Otherwise, you will need to schedule an in-person visit to establish care. When its time to renew your card, you can chose to have an in-person or telehealth/virtual appointment.

Complete your 329 application on the Hawaii Dept of Health Website prior to your appointment. Bring your medical records verifying your qualifying condition. We will review your qualifying condition and answer any questions you have about cannabis use during your appointment.

Application assist available for additional $25 fee

If you received your previous card from our provider (Dr. Stephanie Dent DNP, APRN), schedule your renewal with us as well! You can renew for 1 or 2 years (as long as you are renewing for the same condition you were previously evaluated for). Appointments can be in-person or via telehealth. Call us prior to submitting application if you have questions about whether you qualify for a one or two year card upon renewal.
Application assist available for additional $25 fee

Get a holistic workup to evaluate your condition. This option is for you If you have not previously had your qualifying condition evaluated by a medical provider. Schedule an appointment with us to diagnose your medical condition and recommend a holistic treatment plan. This may require obtaining labs/imaging/ and prior medical records. *Create your 329 application prior to your appointment* 60 min- $250 plus travel fee if applicable
