Currently, the state of Hawaii has legalized the medical use of cannabis for patients with qualifying medical conditions. To obtain Hawaii cannabis cards, patients can undergo a cannabis evaluation for the following conditions:

- Amyotrophic Lateral Sclerosis (ALS)
- Multiple Sclerosis
- Severe/Persistent Muscle Spasm, including Crohn's disease
- Glaucoma

- Epilepsy/Seizures
- PTSD
- Other conditions as determined necessary by your primary care doctor.
Many patients use medical cannabis as an alternative, or in addition to, pharmaceutical medications to assist with symptom relief caused by their qualifying health condition. Cannabis is typically well-tolerated, and many patients are pleased to discover a natural and effective alternative to relying solely on pharmaceuticals. We are happy to assist patients through cannabis evaluations and provide guidance on obtaining Hawaii cannabis cards to determine if medical cannabis is appropriate for you and your condition.
We have assisted hundreds of patients in navigating the process of obtaining their Hawaii cannabis cards and medical cannabis. Our staff is skilled in helping you create your application with the Hawaii Department of Health and will conduct a cannabis evaluation using your existing medical records to determine your eligibility for medical cannabis. If you haven't had your condition diagnosed by a doctor yet, we can help with this process as well (please note this will require a longer appointment and may involve obtaining your medical records, labs, or imaging).
We offer mobile/ house call visits and telehealth appointments.
House calls are based on provider availability.
Initial appointments can be conducted virtually if you are able to provide medical records verifying your condition at the time of your appointment. Otherwise, you will need to schedule an in-person visit to establish care. When it's time to renew your card, you can choose to have an in-person or telehealth/virtual appointment.

5-10 miles from Wailuku- $50
10-25 miles from Wailuku- $75
>25 miles from Wailuku- Please call us for a quote

Complete your 329 application for Hawaii cannabis cards on the Hawaii Dept of Health Website prior to your appointment. Bring your medical records verifying your qualifying condition for a cannabis evaluation. We will review your qualifying condition and answer any questions you have about medical cannabis use during your appointment.

If you received your previous Hawaii cannabis cards from our provider (Dr. Stephanie Dent DNP, APRN), be sure to schedule your renewal with us! You can renew for 1 or 2 years, as long as you are renewing for the same condition you were evaluated for during your cannabis evaluation. Appointments can be conducted in-person or via telehealth. Please call us before submitting your application if you have questions about your eligibility for a one or two year card upon renewal.

Get a holistic workup to evaluate your condition through a cannabis evaluation. This option is for you if you have not previously had your qualifying condition assessed by a medical provider.
